We maintain a Preferred Provider Network in the U.S. though international partners. In the United States and Canada, provider choices and reimbursement assessment will be based upon four tiers of provider network type:

1.  Preferred Provider Network - 0% Co-pay

This Tier consists of all First Health Providers as well as other preferred providers designated by the Company and listed on the website

2.    Out-Of-Network (when network provider was available within a 30 mile radius) - 20%   Co-pay, no out-of-pocket maximum and benefits payable are limited to the Usual and Customary Charges for the services received.  If a discount is negotiated with the provider, the savings thus realized will be passed on to the Insured Person.

3.   Out-of-Market Area - No Network providers located within 30 Mile Radius - 0% Co-pay

Clearly, members should select a Preferred Provider whenever possible. GBG / TieCare and CMN have an extensive Network of physicians and hospitals. There are over 50,000 providers in the Network. The US Network includes all the First Health providers, which our members have been using for several years, supplemented by even greater choice with the addition of a number of proprietary Networks.
